Yet another corrupted nexus no rewards bug

This is slightly different than the other threads here I've seen.

I have the "complete a Tier 12+ corrupted nexus". I took on a T14 Sun Temple. I died to the first boss once. When I killed it, it said the map was completed. It never spawned the second boss, and I never got the skill book, nor did the quest advance. It appears to count as completed though because the surrounding maps are now cleansed.

not a bug...die at boss once and content is gone.
Map is completed, not the nexus.
